Update of OpenSHMEM 1.0 Reference Library and Test Suite at SC'12

New releases of the OpenSHMEM 1.0 reference library and the test suite, both revision "d", for implementers are now available at SuperComputing.

N.B. we have skipped release "c" of the test-suite to synchronize the version names for future joint updates.
